To apply to St. Anne Catholic Grade School:

All new families must fill out the Application for Admission form.
If you wish to apply for our Pre-K or Three-year-old program, please fill out the Application for Admission form as well as the specific program application. Please see Preschool/Pre-K tab on the left to download additional documents.
At time of admission, all new families must have a copy of their child's:
- birth certificate
- baptismal certificate
- immunization record
- copy of their current report card (if applicable)
All families must fill out an Emergency Form and turn it into the office as soon as possible. This form should be current in case it is necessary to contact parents during the school day. If parents cannot be contacted, it is important to have other persons on file to contact.
If you are interested in registering for Extended Day (Latchkey,) please print out the application and turn it into the office or the Latchkey room. Latchkey hours are from 7 a.m. - 9 a.m. and 4 p.m. - 6 p.m.

Release for Dispensing Medicine Form: Please note that the Archdiocese of Detroit states that the following steps need to be followed:

Please send all medication-including cough drops-to the school office.
Please have your doctor instruct us with the name, dose, and time that the medication should be dispensed on his/her letterhead with his/her signature.
Please fill out the Release for Dispensing Medication Form as well as the doctor's instructions and send it to the office.

The purpose of this policy is to keep all of our students safe and healthy.
