St. Anne Catholic Grade School

St. Anne Catholic Grade School admits students of any race, nationality, and ethnic origin to all rights and privileges, programs, and activities generally accorded to students of this school. Preschool students should be “potty trained” and have received all state required immunizations. Proof of immunizations must be on file in the school office before the first day of school. During the enrollment process, a birth certificate, an updated physical, and a current immunization record must be on file. Please provide a baptismal certificate if available. A child information card, licensing notebook form and a family emergency form must be on file by the first day of school.
